Using Google Forms To Create A Survey

Google Docs contains a "forms" features that allows Google Apps users to create and administer surveys. The survey should go to users of Carthage's Google system, not to outside email addresses.
To get started, first go to Google Docs by logging in to your Google e-mail then clicking the "Documents" link at the upper left corner. Once in Google Docs, follow these instructions to create your form:
1. Click the "Create New" button at the upper left, then click "Form"
2. Fill in the title of your form
3. If you want to collect the username of the respondents, check the appropriate box at the top of the form
4. Fill out the description for the form, as needed
5. The first question is automatically included. Fill out the title of the first question and the text to go with it (This could be a "text" question asking for the name of the respondent.
6. Click on the drop-down list for the Question Type (such as Multiple Choice, scale, etc)
7. If you want the question to be required, check the box next to "Make this a required question"
8. Enter the options for the question, as needed. When finished with the question, click "done" then go to question 2
9. You can add a question to your form by clicking "+add itme" in the top left of the screen
10. When finished with all the questions, click on the "save" button on the top right.
11. You may then edit more or close out.
12. You can set the "theme" or look of the test form by selecting "theme" on the upper left fo the screen. Scroll through the themes and choose APPLY when you find one you like.
13. When the test form is completed, you can email the form to individuals or established contact lists.
14. Once a user completed the survey, you will receive an email confirmaion in the form of a spreadsheet for easy review
Editing Forms
1. Open the form from your Google Docs
2.Click on FORM in the menu along the menu at the top of the form (Underneath the name of the form)
3. Choose Edit Form.
4. This takes you back the creator version of the form. Follow the directions above to add more questions.
5. You can delete questions by clicking on the trash can icon next to the particular question you want to delete.
Viewing your results:
1. You can view a summary of your results by opening the form from your Google Docs.
2.Click on FORM in the menu along the menu at the top of the form (Underneath the name of the form)
3. Choose Show Summary of Responses.
4. You now can view charts to review your survey results.
